Studio Rules & Policies

Parents and/or Students agree to abide by studio rules and policies. The Studio reserves the right to refuse service based on conduct or behavior.

Attendance & Payment

All students must check-in before each class.
Classes must be paid for prior to attending each class.
All students must be registered to take any classes.

Waiver & Release Agreement

I, Parent/Student, acknowledge that participation in dance involves inherent risks, including physical/psychological injury, or pain arising from my own actions, others' actions, or the facility's condition. I voluntarily assume all risks, known or unknown. I hereby release, waive, and discharge Attitude Ballet & Dance Studio, its officers, employees, volunteers, agents, and representatives from any and all liability, claims, demands, or causes of action for injury, death, or property damage. This release includes claims arising from their negligence or fault. I confirm I/my child am/is physically fit and have sufficiently prepared for this activity. I assume responsibility for any injuries sustained. In case of emergency, I authorize the Studio to secure necessary medical treatment and agree to be responsible for all costs. I have read, understand, and fully agree to the terms of this waiver. 

By signing, I understand that I give up considerable future legal rights, including the right to sue. Clicking 'Register' indicates your signing and acceptance of the Waiver & Release Agreement stated on this Registration Form.
